If in the interim, your tax status has changed to resident alien (permanent resident) or became a US citizen since account opening, please select and complete the Internal Revenue Service Form W-9 option provided.
Federal regulations require that we periodically update this information. We request that you complete and electronically sign the appropriate form as soon as possible to avoid receiving this reoccurring notifications:

Failure to update and sign this form may result in the Credit Union having to backup and withhold a percentage of interest earned as required by federal regulations.
